This laser is used to remove a wide variety of pigmented skin lesions. At the conference, Dr. Anterasian performed a Fraxel Repair laser treatment on an actual patient in front of the other physicians who were present.  Approximately thirty physicians attended the conference and as a result of this demonstration, they were able to learn how to perform Fraxel Repair laser treatments. &lt;p&gt;Many people have scars that they find unattractive.  In addition, the presence of the scar may continue to remind the person of an experience from the past that was upsetting such as a cancer treatment, a traumatic injury, or a medically required surgery.  Scars like these can occur on any part of the body.  Other times, the scars are simply the result of an earlier childhood or teenage ailment such as acne or chicken pox.&lt;/p&gt;

&lt;p&gt;Thanks to modern medicine, there are now many ways that scars can be cosmetically improved.  In this month&#039;s newsletter, we will review the different types of scars and the methods available to improve the appearance of unfavorable or unsightly scars.&lt;/p&gt;

&lt;p class=&quot;runOn&quot;&gt;&lt;b&gt;Scars come in several different types.&lt;/b&gt;&lt;/p&gt;
&lt;ul&gt;
&lt;li&gt;&lt;i&gt;Hypertrophic scars&lt;/i&gt;: Hypertrophic scars are raised, firm, thick, and red.&lt;/li&gt;
&lt;li&gt;&lt;i&gt;Atrophic scars&lt;/i&gt;: Atrophic scars are depressions in the skin that result from the loss of collagen.&lt;/li&gt;
&lt;li&gt;&lt;i&gt;Keloid scars&lt;/i&gt;: Keloid scars are raised reddish nodular scars that are quite firm and relatively large.  With a keloid scar, the size of the keloid is actually larger than the size of the original surgery.&lt;/li&gt;
&lt;li&gt;&lt;i&gt;Acne scars&lt;/i&gt;: Acne scars are very common.  Although they may occasionally be hypertrophic or keloidal, they are usually atrophic.  The most common type of atrophic acne scar is referred to as an &quot;ice-pick&quot; scar.  Ice-pick scars are narrow with sharply demarcated tracts that can reach deep into the skin.&lt;/li&gt;
&lt;/ul&gt;

&lt;p class=&quot;runOn&quot;&gt;&lt;b&gt;Options for improving the appearance of scars include the following:&lt;/b&gt;&lt;/p&gt;
&lt;ul&gt;
&lt;li&gt;&lt;i&gt;Steroid Injection&lt;/i&gt; &amp;#8211; Steroid injections are often used to treat hypertrophic scars and keloid scars.  With a series of steroid injections, the inflamed raised scar should flatten out and become less inflamed and less irritated.  Although steroid injections may be used alone, combining steroid injection with laser treatment enhances scar treatment.&lt;/li&gt;
&lt;li&gt;&lt;i&gt;Vbeam Laser&lt;/i&gt;  &amp;#8211; The Vbeam laser is an excellent choice for hypertrophic scars and keloids.  In research studies, there has been significant improvement in scar redness and in reduction of bulk following Vbeam laser treatment.  Generally, after three or four treatments, the scars are flatter and less red.  The Vbeam laser procedure is very well tolerated, with patients only feeling a mild snapping sensation.  There is no downtime after a Vbeam laser treatment.&lt;/li&gt;
&lt;li&gt;&lt;i&gt;Fraxel Lasers&lt;/i&gt; &amp;#8211; The two Fraxel lasers will improve all types of scars and they are the most commonly used lasers to improve the appearance of scars.  The most common scars treated with the Fraxel lasers are atrophic acne scars and surgical scars.  However, even traumatic scars resulting from injury to the skin can be improved with the Fraxel lasers.  Often, Fraxel laser treatment is combined with steroid injection to provide optimum improvement of the scar.&lt;br/&gt;&lt;br/&gt;Patients undergoing Fraxel laser treatment of scars may choose between traditional Fraxel laser treatment and Fraxel Repair laser treatment.  With traditional Fraxel laser treatment, patients undergo a series of 4 or 5 Fraxel laser procedures at monthly intervals, with no downtime, to improve the scar.  The improvement in the scar results from the new collagen that is formed as a result of the laser treatment. This procedure is FDA approved for the treatment of acne scars and surgical scars, and it will also improve all other types of scars as well.&lt;br/&gt;&lt;br/&gt;With Fraxel Repair laser treatment, patients generally need only one or two treatments; however, depending on the size of the treated area, there may be a few days of downtime with Fraxel Repair laser treatment.   The improvement in the scars following treatment with the Fraxel Repair laser results from immediate shrinkage of the scar tissue followed by new collagen growth.  Like the traditional Fraxel laser, the Fraxel Repair will improve all types of scars, including acne scars, surgical scars, keloid scars, and atrophic scars.&lt;/li&gt;
&lt;li&gt;&lt;i&gt;Surgical Scar Revision&lt;/i&gt; &amp;#8211; Occasionally, a scar that healed poorly needs to be surgically removed and stitched using very precise and careful surgical technique.  In properly selected patients, this will result in a much-improved scar.  However, surgical scar revision is only rarely necessary, and nearly all patients can have their scars improved without surgery, using lasers and steroid injections.&lt;/li&gt;
&lt;/ul&gt;

    &lt;p&gt;During the past four years, there have been dramatic and revolutionary advances in laser technology.  By applying these new technologies to the eyelids, it has become possible to rejuvenate and tighten the eyelid skin in nearly all patients without surgery.  Today, surgery only needs to be done on the most severely aged eyelids.&lt;/p&gt;

&lt;p&gt;In order to match the correct eyelid rejuvenation procedure to the right patient, we divide the eyelid skin into three degrees of laxity: mild, moderate, and severe.  Patients who only have mild eyelid laxity are best treated with the eyelid Thermage procedure.  Eyelid Thermage is a procedure that smoothes the texture of the eyelid skin and deceases eyelid hooding and wrinkling. Patients benefit from a single treatment that has no downtime.  Patients who have had eyelid Thermage, which is also known as the “Eyes by Thermage” procedure, report that their eyelid sagging is improved and both the upper and lower eyelids are tighter. The eyes look brighter and appear more open. In addition, the improved smoothness of the skin makes it easier to apply eye make-up.&lt;/p&gt;

&lt;p&gt;Eyelid Fraxel is intended for patients who have developed moderate laxity and crepiness of the skin of the upper and lower eyelids.  The procedure reduces skin crepiness, softens lines and wrinkles, tightens the skin, and produces an overall skin rejuvenation effect.   The eyelid Fraxel procedure utilizes Fractional Resurfacing Technology.  The laser delivers a series of closely spaced laser spots to the skin while preserving normal healthy skin between the laser spots.  This preservation of normal untreated skin between the laser spots results in rapid healing after the treatment. Traditional Fraxel laser resurfacing is generally done as a series of four or five treatments given at monthly intervals.  The eyelid treatment is generally done in conjunction with Fraxel laser treatment of the face.   The procedure has minimal or no downtime.  After completing their treatments, patients can reliably expect the skin to look more youthful and rejuvenated.&lt;/p&gt;

&lt;p&gt;Patients with more severe laxity are best treated with the Fraxel Repair laser.  With the Fraxel Repair, patients obtain significant tightening of their eyelid tissues in a single procedure, which is a welcome advance.  Fraxel Repair eyelid treatment can be done by itself or it may be combined with treatment of the face.  Patients who undergo Fraxel Repair treatment should be prepared for three to five days of healing following the procedure.&lt;/p&gt;

&lt;p&gt;Eyelid Thermage, eyelid Fraxel, and Fraxel Repair all stimulate the production of new collagen in the eyelid skin and promote tissue tightening.  This new collagen is living tissue, and for this reason, the improvement in the eyelids following any of these treatments is long lasting and should persist for years.&lt;/p&gt;

&lt;p&gt;Regardless of which technology is used to treat the eyelids, the rejuvenation may be further enhanced with Botox and dermal fillers.  Botox is used to soften the lines around the eyes (crow’s feet lines).  Dermal fillers such as Juvederm and Restylane are used to soften the depressions and folds that form below the eye (known as the tear trough).  By combining Thermage or either of the Fraxel lasers with Botox and dermal fillers, nearly all of our patients can achieve very natural eyelid tightening and rejuvenation without undergoing a surgical procedure.&lt;/p&gt;

    					&lt;p&gt;We recently completed filming our third season of work for the television show &quot;10 Years Younger.&quot;  Episodes that we participated in will air on The Learning Channel (TLC) on Friday March 21st, Monday March 31st, and Friday April 4th at 1:00 PM.&lt;/p&gt;

					&lt;p&gt;If you have not yet watched the show, the patients on the show undergo a series of non-surgical treatments and enhancements designed to optimize their appearance and help them to look &quot;10 Years Younger.&quot; Doing the show has been a lot of fun.  After watching a few episodes, viewers are able to see all of the non-surgical facial rejuvenation techniques that are currently available.     					&lt;p&gt;What is a cosmeceutical? Cosmeceuticals are skin care products that contain ingredients that influence the physiological functioning of the skin.  When a cosmeceutical is applied to the skin, the biologically active ingredients within the cosmeceutical are absorbed into the skin, resulting in improved skin appearance.  The most commonly used active ingredients in cosmeceuticals are vitamins, herbs, plant extracts (which are referred to as botanicals), and oils.  Regardless of what other facial rejuvenation techniques are used, everyone should have a good cosmeceutical regimen to treat, nourish, protect, and moisturize his or her skin.&lt;/p&gt;

					&lt;p&gt;The cosmeceutical market has grown rapidly over the past few years as the baby boom generation has gotten older.  This growth has led to the introduction of literally thousands of different skin care products.  Many of our patients have become confused regarding how the different active ingredients in cosmeceuticals help the skin.  In order to better educate our patients about cosmeceuticals, the paragraphs that follow will review the most common ingredients that are used in today&#039;s cosmeceuticals and how they help the skin look better and more refreshed.&lt;/p&gt;

					&lt;p&gt;&lt;b&gt;Retinoids&lt;/b&gt; &amp;#8211; Retinoids are derivatives of vitamin A and are the most common type of cosmeceutical used today.  In the skin, retinoids act as antioxidants and they neutralize the free radicals that result from exposure to ultraviolet light.  In addition, retinoids increase cellular turnover, stimulate a beneficial thickening of the epidermis, and promote the removal of excessive skin pigment.  The use of retinoids leads to the deposition of new collagen in the skin. As a result of these actions, retinoids are known to reduce wrinkles, decrease skin laxity, improve skin color, and produce a smoother skin surface.&lt;/p&gt;

					&lt;p&gt;Tretinoin (retinoic acid, Retin A, Renova) and tazarotene (Tazorac, Avage) are two of the most commonly used retinoids.  Skin care products that contain either of these two ingredients are available by prescription.  Retinol is another frequently used retinoid.  It is chemically identical to Vitamin A.   Because it is a naturally occurring vitamin, skin care products that contain Retinol do not require a prescription and are available over-the-counter.&lt;/p&gt;

					&lt;p&gt;&lt;b&gt;Hydroxy acids&lt;/b&gt; &amp;#8211; Hydroxy acids are another very commonly used active ingredient in cosmeceuticals.  They are classified as either alpha hydroxy acids (AHAs) or beta hydroxy acids (BHAs) depending on their molecular structure.  Commonly used alpha hydroxy acids are glycolic acid, citric acid, and lactic acid; the most commonly used BHA is salicylic acid.&lt;/p&gt;

					&lt;p&gt;The AHAs used in cosmeceuticals have been shown to produce younger looking skin.  Cellular turnover is increased, and the skin appears smoother and more uniform in color.&lt;/p&gt;

					&lt;p&gt;Salicylic acid is used as a skin conditioner and is especially useful in patients with oily skin and acne.  It is considered to be one of the most effective acne treatments.  Salicylic acid would be of particular benefit to individuals who have both aging skin and acne.&lt;/p&gt;

					&lt;p&gt;&lt;b&gt;Antioxidants&lt;/b&gt; &amp;#8211; Although it may seem obvious, an antioxidant is a chemical that prevents living tissues from becoming &quot;oxidized&quot; and undergoing a chemical change. In the body, highly reactive substances called free radicals are constantly being produced. Free radicals can quickly damage the body&#039;s own cells. In living tissues, antioxidants remove free radicals before they can damage normal tissues.&lt;/p&gt;

					&lt;p&gt;From the dermatology perspective, exposure to ultraviolet radiation from the sun and other sources will result in the production of free radicals. Several studies have suggested that antioxidants help protect the skin from the damaging effects of the free radicals that result from exposure to ultraviolet light.  The most common antioxidants that are used in cosmeceuticals are Vitamin C, Vitamin E, Panthenol, Lipoic Acid, Ubiquinone, and Niacinamide.&lt;/p&gt;

					&lt;p&gt;&lt;b&gt;Depigmenting Agents&lt;/b&gt; &amp;#8211; Depigmenting agents are skin-lightening ingredients.  They remove excessive melanin pigment from the skin.  A depigmenting agent works best on &quot;epidermal melanin,&quot; which is melanin in the top layers of the skin.  When the excessive melanin is located deeper in the skin in the dermal layer, a depigmenting agent may not be as effective.&lt;/p&gt;

					&lt;p&gt;Hydroquinone is the most commonly used depigmenting agent; it works by decreasing the production of melanin pigment. Some physicians also add tretinoin (retinoic acid) and a steroid to the hydroquinone. The tretinoin acts by increasing cellular turnover, resulting in a quicker response to treatment, and the steroid acts by reducing inflammation in the skin.  Tri-Luma cream conveniently combines hydroquinone, tretinoin, and a steroid in one medication, and is available by prescription.&lt;/p&gt;

					&lt;p&gt;Other compounds that are used for skin lightening include azelaic acid, kojic acid, and peels containing alpha hydroxy acids such as glycolic acid.&lt;/p&gt;

					&lt;p&gt;&lt;b&gt;Botanicals&lt;/b&gt; &amp;#8211; With the modern trend toward the use of more natural ingredients, botanicals have become a component of nearly every cosmeceutical product available and form the largest category of active ingredients used today.  An appropriate botanical can be chosen to reduce inflammation, sooth the skin, or act as an antioxidant.  Some of the most commonly used botanicals are prickly pear and aloe vera (for skin soothing), ginkgo biloba, willow herb, and green tea (anti-inflammatory), and goji berry and soy (antioxidants).&lt;/p&gt;

					&lt;p&gt;&lt;b&gt;Peptides&lt;/b&gt; &amp;#8211; A peptide can be thought of as a small part of a much larger protein molecule.  Peptides were developed for use in skin care to either increase the production of collagen or to reduce the breakdown of collagen.  This is important because loss of collagen in the skin is one of the causes of wrinkles.&lt;/p&gt;

					&lt;p&gt;One of the best known peptides used in cosmeceuticals today is referred to as a &quot;pentapeptide.&quot;  This peptide is actually a small fragment of the protein that makes up human collagen.  This &quot;pentapeptide&quot; has been shown to increase collagen production.&lt;/p&gt;

					&lt;p&gt;Matrix Metalloproteinase Inhibitors (MMPIs) &amp;#8211; Matrix metalloproteinases are proteins found in the skin that break down collagen.  They are activated by exposure to ultraviolet light as well as by exposure to air pollution, smoking, and the natural aging process.&lt;/p&gt;

					&lt;p&gt;There are two ways to block the activity of matrix metalloproteinases, and in so doing, help to preserve the skin&#039;s collagen.  The first is with a good sunscreen that protects against ultraviolet light.  The second is with substances that actually block the activity of the matrix metalloproteinases; these substances are referred to as matrix metalloproteinase inhibitors (MMPIs).  Substances that act as MMPIs include derivatives of green tea, tretinoin (retinoic acid), beta-carotene, vitamin E, vitamin C, and flavonoids.  The use of MMPIs in cosmeceuticals is one of the newest and most exciting innovations in skin care, and MMPIs are expected to receive much more attention in the near future.&lt;/p&gt;

					&lt;p class=&quot;sectionEnd&quot;&gt;Cosmeceuticals are formulated and available for purchase from a large number of manufacturers, and their use has become a standard component of modern skin care. Because there are literally thousands of skin care products in the marketplace from which to choose, it can be quite confusing as to which products are best for a particular person.  For this reason, it is ideal for each patient to have his or her skin care program designed by a skin care expert.     &lt;p&gt;By now, most of our patients have become very familiar with Fraxel laser resurfacing of the face. Introduced in 2004 and now in its second generation, the Fraxel laser has become the gold standard for no or minimal downtime laser skin resurfacing. After a series of Fraxel laser treatments to the face, patients can reliably expect their skin to show improvement in fine lines, brown spots, acne scars, facial scars, and overall skin texture.&lt;/p&gt;

&lt;p&gt;However, you may not be aware that the Fraxel laser is increasingly being used for skin rejuvenation in locations other than the face. In recent months, many of our patients have come to the office requesting rejuvenation of the skin of their neck and chest; other patients have requested rejuvenation of their hands and arms.
